Angela Ashton, OTR/L, CCM Founder
With over 30 years of clinical experience as an Occupational Therapist and Certified Case Manager, Angela has dedicated her career to the well-being of the aging population across the entire continuum of care—from Acute Care and Rehabilitation to Skilled Nursing and Home Health.
In 2020, driven by a vision for a more holistic, person-centered approach, she founded LiveWell Elder Care Management. Her mission is simple yet profound: to uphold the dignity and independence of her clients, providing expert and personalized care that is rooted in community and delivered with professionalism, compassion, and joy.

Angela believes that exceptional care never happens in a vacuum. She serves as a strategic hub, expertly uniting a network of community partners—including medical providers, elder law attorneys, financial planners, facility directors, and home care agencies. Our team
Cathy Warren, PT, DPT, CMC
Cathy Warren, PT, DPT, CMC, is a highly credentialed Physical Therapist and a dedicated member of our team, contributing 40 years of clinical experience to our practice. She has been a vital asset at LiveWell Elder Care Management since 2022.
Cathy’s extensive education includes a BS in PT from Northeastern University and both a Master’s and a Doctorate in Physical Therapy (DPT) from UMass Lowell. Further demonstrating her commitment to the field, Cathy is also a Certified Care Manager (CMC), a highly respected certification from the National Academy of Certified Care Managers (NACCM).
Her career has spanned the full spectrum of geriatric settings: acute care hospitals, skilled nursing facilities, outpatient clinics, and home health. This breadth of experience is invaluable, allowing her to expertly assess client safety, smoothly coordinate transitions of care between different environments, and ensure clients maintain optimal mobility and independence in their daily lives.
When not serving our clients, Cathy enjoys spending time with her family, engaging in activities like gardening and skiing, and relaxing with a good book.
Sheri LaBree Boyce, LICSW, CAGS
Sheri LaBree Boyce, LICSW, CAGS, is a licensed social worker with a Master’s Degree in Social Work from Salem State University, specializing in health and mental health. She also holds two certificates of advanced graduate studies. With over 20 years of experience, Sheri is passionate about working with older adults and their families.
Before joining LiveWell Elder Care Management, Sheri worked in various settings, including an inpatient psychiatric hospital, nursing homes, the Executive Office of Elder Affairs, and Care.com. At Care.com, she managed a work-life benefit program for companies like Harvard University and Starbucks. During her time at the Massachusetts Executive Office of Elder Affairs, Sheri helped manage the statewide Long Term Care Ombudsman Program, advocating for residents in nursing and rest homes. This experience has made her an expert in nursing home care.
Sheri has presented seminars on senior care topics at NVIDIA, Twitter, MIT, Netflix, and Harvard University. She is also an LGBTQ+ ally and has collaborated with organizations like the Trevor Project to provide workplace inclusivity training.
Having cared for several family members, including her mother, Sheri brings both personal and professional experience to her work with older adults and their caregivers. In her free time, Sheri is writing a book for caregivers about toxic relationships and caregiving, tentatively titled “Caregiving for the Parent Who Wasn’t There for You.” She also enjoys day trips with her two stepchildren, spending time with her cats, cooking, and visiting new coffee shops.
Katherine Ware, OTR, CDP, CCM
Katie is a seasoned Occupational Therapist with more than 25 years of experience supporting individuals in their homes and communities. She is deeply committed to assisting those facing cognitive challenges, particularly individuals living with dementia, and is recognized for her integrative, collaborative, and compassionate approach to care.
Katie brings a strong clinical foundation from her work in skilled nursing facilities and home health care, where she has managed a wide range of medical, cognitive, and functional needs. She is highly skilled in navigating the healthcare system, coordinating medical and community services, and guiding clients and families through complex decision‑making. Her work emphasizes continuity of care, safety, and effective aging‑in‑place strategies.
A lifelong learner, Katie began her training at Worcester State University, earning her degree in Occupational Therapy. She has since expanded her expertise as a Certified Dementia Practitioner, Certified Case Manager, and certified MoCA rater, enabling her to assess and support clients with diverse cognitive needs. As an Advanced Professional with the Aging Life Care Association®, she brings a holistic, informed, and client‑centered perspective to every case she manages.
Katie is a proud military spouse and lives in Templeton, MA with her husband and their three boys.

What is an Aging Life Care™ Manager?
An Aging Life Care Manager is dedicated to helping clients age well while also supporting quality of life for family members. This phase of a family’s journey together is often complicated. There is more to do than there is time. Relatives may live far away. Relations can become strained.
Our professional training and knowledge of best practices can ease the journey.
Every family is unique. As Aging Life Care Managers, we apply our insights and skills in 8 key areas to provide you with a positive care plan tailored to your family’s individual needs and strengths.
We offer expertise regarding:
- Health and disability: Through assessments and experience, we can help you understand what your loved one does well, and in what areas he or she could use additional help.
- Finances: A review of the current monetary situation will result in a plan to make resources stretch as efficiently as possibly while maintaining the desired level of comfort, dignity and safety.
- Housing: Based on the assessments, recommendations can be made for optimal living situations now, with a view to likely changes in the foreseeable future.
- Family: Sibling disagreements and old family history often make the aging journey difficult. Our training provides for skilled facilitation of family meetings and creation of a plan that takes everyone’s needs into account.
- Local Resources: There are programs available to help, but it’s difficult to know which one is the most appropriate for your situation. Save time and money by letting us objectively evaluate the situation and connect you to the resources best suited to your needs.
- Advocacy: Our comprehensive knowledge of the disjointed health care system allows us to monitor quality and lobby for the care and treatment your family wants.
- Legal: Let us help you identify issues and get key paperwork in order.

How we are different
What makes an Aging Life Care Manager stand apart from the others is the commitment to stay focused on the needs of the client and family. Others may offer services for less, but what you receive cannot help but be influenced by the payment structure.
Hospital- or insurance-based case managers are employees of the health care system. They are good professionals. Their focus, however, is to address the issues of patients within the institution and to make efficient use of services for their organization.
We are based in the community and work with aging adults wherever they live. We provide continuity of care across all settings. We include the family and focus on improving quality of life for the older adult and their grown children.
Senior advisors are paid referral fees to suggest elder care facilities. As a general rule, they have a more limited sphere of knowledge and do not hold a professional degree needed to give them expertise in the 8 key areas of elder care. Only those facilities willing to pay a finder’s fee will be included in their list of suggestions. There may be a more appropriate option, but it will not be mentioned if the facility does not pay for referrals.
Because we are paid directly by you, we are independent and objective. Our focus is 100% on the needs of your family and your aging loved ones. We apply our considerable skill and years of training to your unique situation.
